Homeless Identification Form

Your child's school will have enrollment identification forms to complete;
Complete the form and give it to your schools registrar;
Your school registrar will contact your school social worker and forward requests to the homeless liaison;
Your information is kept confidential;
Your childs right as a homeless student are included on the enrollment form.
